Project Server 2013 - PDP

Hi All,

We are using Project Server 2013. We need to show a hyper link in Project Information PDP for the project management plan document uploaded in project workspace, how can we do that.

We need to link  the issues and risks with project tasks. In 2007 there will belink item button in issues and risks form, but i couldn't find in 2013

In the PDP can we have two input box in a row. i.e. we need to show the have the Project health column with its drop down value and next to that another input box for comments as marked red below. Is it possible to achieve in PDP.
